Accident Insurance Scheme For the 2004/2005 season, the League has increased the insurance cover for your players to include broken bone benefit (arms and legs only). A copy of the insurance can be found at he rear of this handbook. The following claims guidance notes have been issued by the insurers, Sportsguard and are to be followed in the event of a claim. All claims must be reported within 30 days or as soon as possible, so as Underwriters can be notified. All claim forms must be returned within 60 days of the accident. The claim form must be completed in its entirety. If not, the claim form will be returned to the player concerned for completion. Yours clubs premium must be paid up fully of if you are paying by instalments your payments must be up to date at the time of the incident if not underwriters can void the insurance. In the event of a claim a photocopy of the insurance schedule must be attached with the claim form. Upon the claim form being completed the player must provide proof of employment. Three computerised wage slips or a letter of contract from his employer stating his annual salary. This point is very important to your player because if he cannot provide this proof he will be deemed as unemployed and his benefit will drop 50% of the benefit up to a maximum of £20.00. If your player is self employed he must provide a copy of his certified accounts for the past 12 months or a letter from his accountant stating what his income is. The employment form must be completed by the players employer with a contact point so as underwriters can verify the players claim. Copies of sick notes must be forwarded with the claim. If interim payments are required you must state separately that the player concerned requires this service. Please also note benefit can only be paid in arrears, this means up to a current sick note date. No benefit can be paid out for the future. If your player spends time in hospital he must provide the necessary documentation to prove how many completed days he was an overnight resident. Please note that the broken bone benefit is payable for ARMS and LEGS only subject to a solid Plaster Cast and excluding the hands and feet.
